Health Sciences Scholarship Staff Application Form

The purpose of this scholarship is to assist an HDMH staff member who wishes to pursue further education in health sciences. 

Requirements

1. The applicant must be a staff member for at least one year and who wishes to futher their education.
2. The applicant must be applying to a recognized post-secondary school program that relates directly to his/her assignment or potential assignment within the Huntsville Hospital.
3. Many factors may be used to determine the successful candidate, such as his/her contribution, personal qualities, financial need, academic achievement, and commitment to Huntsville Hospital. The candidate’s participation in community activities, triumphs, goals, aspirations may also be included in the consideration. To give evidence of some of these attributes, the applicant is asked to:

• include three letters of reference from people (other than family members) who can attest to the qualities the candidate possesses to support the scholarship request, and
• submit no more than a one-page essay, explaining candidacy for the award.

Selection Process

The Executive Scholarship Committee of the Huntsville Hospital Auxiliary will make the final selection based upon the above criteria as presented by the applicant, the content of the reference letters and essay submitted, and the use of a common scoring sheet when looking at application forms. The committee may call upon managers or references for more information. Once a decision is made, applicants will be notified.

Annual Value

The number of scholarships and the amount of each award shall be at the discretion of the Huntsville Hospital Auxiliary and will be reviewed each year. The individual scholarship will be for $1,000.
